Prolactin Management on Deca, NPP & Trenbolone

You're running Deca alongside testosterone. Week 5, nipple sensitivity develops. You check your bloodwork — estrogen is 28 pg/mL, perfectly in range. You raise your Anastrozole dose anyway. It gets worse, not better. The problem is that you've been managing the wrong hormone.

Nipple sensitivity and gynecomastia development on 19-nor compounds (Nandrolone, Trenbolone) are frequently prolactin-mediated, not estrogen-mediated. A standard aromatase inhibitor does nothing for elevated prolactin. This guide explains the mechanism, how to identify the source, and the correct management protocol.

What Are 19-Nor Compounds?

19-nor compounds are anabolic steroids where the 19th carbon of the testosterone molecule has been removed. This structural change dramatically alters the compound's behavior:

  • Nandrolone (Deca Durabolin, NPP): Highly anabolic, joint-friendly, low androgenic activity, significant progestin activity

  • Trenbolone: Extremely anabolic (5Ɨ the anabolic rating of testosterone), no aromatization, extremely high progestin activity, and direct prolactin-stimulating effects

The progestin activity — binding to progesterone receptors — is what links 19-nor compounds to prolactin elevation. Progesterone receptor activation in the pituitary can stimulate prolactin secretion through a pathway entirely separate from estrogen.

Prolactin vs Estrogen Gyno: How to Tell the Difference

Both estrogen and prolactin can cause gynecomastia and nipple sensitivity. Getting the diagnosis right determines the correct treatment.

Test prolactin at baseline (before cycle), at weeks 4–6, and post-cycle to confirm recovery. Blood draw should be done without significant stress immediately beforehand — physical or psychological stress transiently elevates prolactin.

Cabergoline: The Primary Treatment

Cabergoline (brand name Dostinex) is a dopamine agonist — it activates dopamine receptors in the pituitary, which directly suppresses prolactin secretion. It is the most effective and specific prolactin management tool available.

Bromocriptine: An Alternative

Bromocriptine is an older dopamine agonist that also suppresses prolactin. It is significantly cheaper than Cabergoline but has a notably worse side effect profile: nausea and vomiting are common, particularly at the start, and the effective dose needs to be titrated up slowly.

Standard Bromocriptine approach if Cabergoline is unavailable:

  • Start at 1.25 mg/day (half a 2.5 mg tablet) with food

  • Increase to 2.5 mg/day after 1 week if tolerated

  • Maximum for prolactin management: 2.5 mg/day

Cabergoline is preferred whenever accessible due to better tolerability and twice-weekly rather than daily dosing.

Vitamin B6 (P5P): The OTC Option

Pyridoxal-5-phosphate (P5P), the active form of Vitamin B6, has a modest prolactin-lowering effect through enhancement of dopamine synthesis. It is not as potent as Cabergoline but is appropriate for:

  • Low-dose Nandrolone use where significant prolactin elevation is not expected

  • Preventive support when a dopamine agonist isn't warranted at the cycle dose

  • Stacking with Cabergoline for additional support

Dose: 100–200 mg/day P5P (not regular pyridoxine — use P5P specifically, the active form that doesn't require liver conversion). Evidence is modest but risk is low at these doses.

Trenbolone: A Higher Prolactin Risk

Trenbolone deserves specific mention because its prolactin effects are more pronounced than Nandrolone's and occur through an additional mechanism — Trenbolone may directly stimulate prolactin receptors, not just through progestin activity. This means:

  • Prolactin elevation on Tren is more consistent and more rapid than on Deca

  • Prophylactic Cabergoline from the first week of any Trenbolone cycle is strongly recommended by experienced users

  • Even with Cabergoline, run bloodwork at weeks 4–6 to confirm adequate prolactin management

Trenbolone is not an appropriate compound for users still learning to manage estrogen on testosterone alone — the prolactin management requirement, combined with its lack of aromatization (no estrogen production at all, which can cause its own issues), makes it an advanced compound requiring experience with hormonal management.

Post-Cycle Prolactin Normalization

After stopping a 19-nor compound, prolactin typically normalizes within 4–8 weeks. Continue Cabergoline for 1–2 weeks after stopping the 19-nor compound, then taper off. Check prolactin at your post-PCT bloodwork (4 weeks post-PCT) to confirm normalization alongside testosterone, LH, and FSH recovery.

Frequently Asked Questions

Can I manage prolactin with Nolvadex instead of Cabergoline?

Nolvadex blocks estrogen receptors at breast tissue and can reduce gyno symptoms regardless of whether the cause is estrogen or prolactin — it's a useful adjunct for the nipple sensitivity symptoms specifically. However, it does not lower prolactin levels. Cabergoline addresses the root cause; Nolvadex manages the tissue-level symptom. Use both if gyno is actively developing.

My prolactin is elevated but I have no symptoms. Do I still treat it?

If prolactin is moderately elevated (18–30 ng/mL) without symptoms, beginning low-dose Cabergoline (0.25 mg twice weekly) is a reasonable preventive step — it stops the elevation before symptoms develop. If prolactin is significantly elevated (above 40 ng/mL), treat regardless of whether symptoms have appeared yet.

Does prolactin affect testosterone suppression?

Chronically elevated prolactin can suppress LH and FSH independently — which means additional testosterone suppression on top of the direct suppression from the anabolic compound. This is another reason prolactin management matters beyond gyno prevention: maintaining adequate LH/FSH facilitates a faster HPTA restart post-cycle.

Is prolactin elevation possible without 19-nor compounds?

Yes — any significant testosterone suppression can modestly elevate prolactin. Stress, certain medications, and pituitary microadenomas (small non-cancerous tumors) can also elevate prolactin. If prolactin is elevated on a non-19-nor cycle, or elevated years after cycle use, rule out a pituitary cause with a prolactin test off-cycle and a brain MRI if values are very high.
