Proviron (Mesterolone): Free Testosterone, Libido and Hardening

Your bloodwork came back with total testosterone well above range. On paper the cycle is working. In practice you feel flat, your libido is worse than it was before you started, and nothing about the experience matches the number on the page.

Someone tells you free testosterone is the figure that matters and that Proviron fixes exactly this. That is broadly correct, and the mechanism is worth understanding rather than taking on faith — because it also explains why Proviron does almost nothing for muscle growth and why people who expect it to are disappointed.

What Mesterolone Is

Proviron is mesterolone, an orally active derivative of dihydrotestosterone. Its ratings are modest — an anabolic rating around 30–40 against testosterone's 100, with androgenic activity considerably higher than that.

Two structural points define how it behaves:

  • It is 1-methylated, not 17-alpha-alkylated. This is the important one. The methylation at the 1 position is what makes it orally active, and it means Proviron does not carry the hepatotoxicity that defines every other common oral. This is a genuine and unusual advantage.
  • It is a DHT derivative, so it cannot aromatize, produces no water retention, and carries the androgenic side effect profile that comes with that family.

Proviron is not a mass builder and was never intended as one. Clinically it has been used for male hypogonadism and fertility support. In a cycle it is a support compound — it improves how everything else works rather than adding much itself.

SHBG: Total vs Free Testosterone

Sex hormone binding globulin is a protein that binds circulating sex hormones and holds them inactive. Bound testosterone cannot act at the androgen receptor. Only the free fraction — typically around 2% of total in a healthy man — is biologically available.

This is why total testosterone can mislead. Two men with identical total testosterone and very different SHBG have completely different amounts of usable hormone. High SHBG means a high total reading and a low functional level, which produces exactly the picture described at the top: good numbers, poor experience.

Proviron binds SHBG with high affinity — higher than testosterone does. It occupies the binding sites, displacing testosterone and pushing it into the free fraction. Nothing new is added; existing hormone is liberated.

The practical result is that your current testosterone dose behaves as though it were higher. More strength, better libido, a stronger androgenic effect overall, without touching the injectable.

The Libido Effect

This is the reason most people run Proviron, and it is the effect it delivers most reliably.

On-cycle sexual dysfunction is common and has several possible causes — crashed estrogen, elevated prolactin, high SHBG, blood pressure. Proviron addresses the SHBG route directly, and its strong androgenic activity contributes independently. For many people it resolves the problem within a week or two.

The important caveat: Proviron fixes the SHBG cause. It does not fix the other causes. If your libido is gone because you over-dosed an aromatase inhibitor and crashed your estradiol, Proviron will not help, and it may make things marginally worse by further reducing estrogenic activity. If prolactin is elevated from a 19-nor, Proviron does nothing for that either.

Diagnose before you medicate. The full differential is covered in the guide to erectile dysfunction on cycle.

Mild Anti-Estrogenic Activity

Proviron binds the aromatase enzyme, competing with androgens for it, which reduces conversion to estrogen. It also binds SHBG in a way that alters the free estradiol picture.

This is real but modest. At 50 mg daily alongside a moderate testosterone dose, some people find they need less of their aromatase inhibitor. On a high-dose aromatizing cycle, Proviron will not control estrogen on its own and should not be treated as an AI substitute.

Use it as a contributor, keep a proper AI available, and dose that AI on bloodwork. The differences between the three inhibitors are covered in the aromatase inhibitor comparison.

The Hardening Effect

Proviron contributes a modest visual improvement — slightly drier, harder-looking muscle. The mechanism is the combination of strong androgenic signalling, no aromatization, and reduced estrogen-driven water retention.

The same precondition applies as with every hardening compound: it only shows at low body fat. Above roughly 12% there is nothing for the effect to reveal, and people running it in that state conclude it does nothing.

The effect is real but considerably weaker than Masteron's. If cosmetic hardening is your primary goal, Masteron is the stronger tool — see the Masteron guide.

Dosing

GoalDaily doseDurationNotes
Libido and wellbeing support25 mgFull cycleEffective for most people at this dose
SHBG displacement and hardening50 mgFull cycleThe common dose
Maximum75–100 mgNot recommendedAndrogenic side effects rise without much added benefit

The half-life is around 12 hours, so splitting into two doses gives steadier levels, though once daily is adequate at 25 mg.

Unlike the 17-aa orals, Proviron is run through the entire cycle rather than pulsed for four to six weeks. There is no hepatic clock forcing a short run. Its value is continuous support, so it is typically started with the cycle and stopped with it.

Proviron vs Masteron

Same DHT family, overlapping mechanisms, different delivery and different jobs.

If you want the SHBG and libido benefit, Proviron gives you most of it orally, cheaply, without another injection. If you want the visual change for a show, Masteron is the stronger compound. Running both is redundant — they compete for the same mechanism.

Side Effects

Proviron is well tolerated, with the caveat that "well tolerated" here means the absence of liver and estrogen problems, not the absence of androgenic ones.

  • Hair loss. The main concern. As a DHT derivative it accelerates androgenetic alopecia in anyone predisposed, and finasteride offers no protection against a DHT derivative.
  • Acne and oily skin. Common, consistent with the androgenic profile.
  • Prostate effects. Worth genuine consideration for older users. If you have existing urinary or prostate symptoms, this is a compound to discuss with a doctor.
  • Suppression. Mild but present — Proviron does suppress LH to some degree. It is not the "non-suppressive" compound it is sometimes described as, though the effect is small next to the injectables it accompanies.
  • Lipids. Some HDL suppression, milder than the 17-aa orals.
  • Liver. Not hepatotoxic in the 17-aa sense. This remains its standout advantage among orals.

Frequently Asked Questions

Will Proviron build muscle?

Not meaningfully on its own. Its anabolic rating is low and it was never designed as a mass builder. What it does is make the rest of your stack work better by raising free testosterone, which can translate into more strength and better training output. Judge it by how the cycle feels and performs, not by the scale.

Can I run Proviron on its own?

It is possible and some men use low doses for libido without a full cycle, but the results are modest. Proviron works by liberating testosterone that is already there — if your testosterone is at natural baseline, there is much less to liberate. Its value scales with what it is displacing.

Does Proviron replace an aromatase inhibitor?

No. Its anti-estrogenic effect is genuine but mild, and it cannot control estrogen on a high-dose aromatizing cycle. It may reduce how much AI you need. Keep a proper inhibitor available and dose it on sensitive estradiol bloodwork.

Is Proviron safe for the liver?

It is not 17-alpha-alkylated, so it does not carry the hepatotoxicity of Dianabol, Anadrol, Winstrol or Turinabol. This is unusual for an oral and is one of the main reasons it can be run for a full cycle. It still warrants normal bloodwork, but liver enzymes are not the concern here.

Will it lower my SHBG on bloodwork?

Measured SHBG may not drop dramatically, because Proviron occupies the binding sites rather than reducing the protein itself. What changes is free testosterone, which rises. Test free testosterone alongside total and SHBG — free is the number that reflects what Proviron is doing.
