You are eight weeks into a cycle with more testosterone circulating than at any point in your life, and you cannot perform. It is humiliating, it feels like it should be impossible, and nobody in your gym will discuss it seriously beyond a joke about Deca.
It is common. It is almost always fixable. And it is almost never a testosterone problem ā which is why adding more testosterone, the instinctive response, tends to make it worse. This guide covers the five actual causes, how to tell which one you have, and where PDE5 inhibitors fit into the answer.
The Paradox
Erectile function is not a simple function of how much testosterone you have. It depends on a set of systems working together: adequate estradiol, controlled prolactin, sufficient free testosterone, healthy vascular function and a nervous system that is not in a permanent state of arousal.
A cycle disturbs several of those simultaneously. So a man with a total testosterone reading five times the upper reference limit can have worse erectile function than he did untreated, and the reading itself tells him nothing about why.
Work through the five causes below in order of likelihood before reaching for a PDE5 inhibitor.
Cause 1: Crashed Estrogen
The most common cause by a wide margin, and the most reversible.
Men need estradiol for normal sexual function. It contributes to libido at the central level and supports the nitric oxide pathway that produces an erection. Suppress it too far and both fail.
This happens constantly because aromatase inhibitors get taken at breast cancer doses on fixed schedules. A man takes 1 mg of Anastrozole because his nipples felt sensitive, crashes his estradiol into single digits, and develops erectile dysfunction that he then attributes to the cycle rather than the pill.
The tell: dry aching joints, a flat depleted look, low mood and disturbed sleep alongside the sexual symptoms. If you are taking an AI and you have those, stop the AI. Function usually returns within a week.
The full picture on dosing inhibitors properly is in the aromatase inhibitor comparison.
Cause 2: Elevated Prolactin
The classic "Deca dick", though Trenbolone produces it faster and more consistently.
The 19-nor compounds have significant progestin activity, which can drive prolactin up. Elevated prolactin suppresses libido and erectile function directly, and it does so with estradiol sitting perfectly in range ā which is exactly why raising an AI dose fails and why people go round in circles.
The tell: you are running Deca, NPP or Trenbolone, estradiol is normal on bloodwork, and there may be nipple sensitivity without any water retention.
The fix is Cabergoline, typically 0.25 mg twice weekly, alongside a prolactin test to confirm. Full protocol in the prolactin management guide.

Cause 3: High SHBG and Low Free Testosterone
Total testosterone is not the usable figure. Sex hormone binding globulin holds most of your circulating testosterone inactive, and only the free fraction reaches the receptor.
Certain compounds and individual variation push SHBG high enough that a man with an impressive total reading has mediocre free testosterone. The number looks excellent and the experience does not match it.
The tell: high total testosterone, high SHBG, unremarkable free testosterone. No joint pain, no nipple symptoms.
The fix is to displace testosterone from SHBG rather than to add more testosterone. Proviron does this directly ā see the Proviron guide.
Cause 4: Blood Pressure and Vascular Function
An erection is a vascular event. Anything that impairs endothelial function impairs it.
Cycles raise blood pressure through water retention, adverse lipid changes and rising haematocrit. Thicker blood, stiffer vessels and impaired nitric oxide signalling all work against you, and this cause tends to build gradually rather than appearing overnight.
The tell: elevated home blood pressure readings, high haematocrit, running harsh orals or a high-dose cycle. Often accompanied by headaches and poor cardio tolerance.
The fix is to address the blood pressure itself ā cardio, sodium control, omega-3, and medical management if readings stay high. This is also the one cause where a PDE5 inhibitor treats something real rather than masking a hormonal problem, since these drugs improve endothelial function directly.
Cause 5: Psychological and Sleep-Driven
Underrated and genuinely common, particularly on Trenbolone.
Chronic sleep deprivation suppresses sexual function on its own. Add the anxiety and irritability that some compounds produce, then add the performance anxiety that follows the first failed attempt, and you have a self-sustaining loop that no amount of bloodwork will explain.
The tell: bloodwork is clean across the board, sleep has been poor for weeks, and the problem is situational rather than constant ā morning erections still occur.
The presence of morning erections is a useful diagnostic. If you still get them, the vascular and hormonal machinery is working, and the problem is more likely psychological or situational. If they have disappeared entirely, the cause is physiological.
The Diagnostic Panel
Do not guess. One blood draw separates all five:
| Marker | What it rules in or out |
|---|---|
| Sensitive estradiol (LC-MS) | Crashed estrogen. Standard immunoassay is unreliable in men ā insist on the sensitive version. |
| Prolactin | 19-nor prolactin elevation. Draw without acute stress beforehand. |
| SHBG | High SHBG binding up your testosterone |
| Total and free testosterone | Free is the meaningful number; total alone misleads |
| Full blood count (haematocrit) | Blood viscosity contributing to vascular impairment |
| Lipid panel | Endothelial risk |
| Blood pressure (home, over a week) | Not a blood test, but the most actionable measurement on this list |
Interpretation shortcut: aching joints and dry flat look ā estrogen. Nipple sensitivity with normal estradiol on a 19-nor ā prolactin. Great total, mediocre free ā SHBG. High readings and headaches ā blood pressure. Everything normal and morning erections intact ā sleep and stress.
Tadalafil
A PDE5 inhibitor with a half-life around 17ā18 hours, which gives it an effective window of roughly 36 hours ā considerably longer than the alternatives.
| Protocol | Dose | Notes |
|---|---|---|
| Daily low-dose | 5 mg daily | Steady levels, spontaneity restored, plus blood pressure and endothelial benefit |
| On demand | 10ā20 mg | Take 1ā2 hours before; effect persists into the following day |
The daily low-dose protocol is what most people on cycle end up preferring. Beyond the primary effect, tadalafil modestly lowers blood pressure and improves endothelial function ā both useful when a cycle is working against you on exactly those fronts. Some users also report better pumps, which is consistent with the vasodilatory mechanism.
Unlike sildenafil, absorption is not meaningfully affected by food.

Sildenafil
Faster onset, shorter duration. Around 30 to 60 minutes to take effect, with a window of four to six hours.
| Tadalafil | Sildenafil | |
|---|---|---|
| Onset | 1ā2 hours | 30ā60 minutes |
| Duration | ~36 hours | 4ā6 hours |
| Daily dosing option | Yes, 5 mg | Not typical |
| Food interaction | Minimal | High-fat meals delay absorption |
| Common side effects | Back pain, muscle aches, headache | Facial flushing, headache, visual tint |
| Best for | Continuous cover; blood pressure benefit | Predictable single occasions |
Sildenafil suits someone who wants a defined window and a fast onset. Tadalafil suits someone who wants the problem to stop being something they have to plan around.

Safety: One Absolute Contraindication
Never combine a PDE5 inhibitor with nitrates. Nitroglycerin, isosorbide, and recreational nitrites ā poppers ā all fall under this. The combination can cause a catastrophic drop in blood pressure. This is not a caution; it is an absolute contraindication and people have died from it.
Also exercise care with alpha-blockers, with significant cardiovascular disease, and if you are already on antihypertensive medication. Seek immediate medical attention for an erection lasting more than four hours ā priapism is a genuine emergency and delay causes permanent damage.
Fix the Cause, Not Just the Symptom
PDE5 inhibitors work regardless of the underlying cause. That is convenient and it is also the trap ā they let you carry on with crashed estrogen or unmanaged prolactin while masking the most obvious signal that something is wrong.
Crashed estrogen is also damaging your joints, lipids and bone density. Elevated prolactin is independently suppressing LH and will slow your recovery in PCT. High blood pressure is a cardiovascular risk that will not announce itself any other way. A tablet that restores function does nothing about any of that.
Use the PDE5 inhibitor while you correct the actual problem, not instead of correcting it.
When to Stop the Cycle
Some situations warrant ending the cycle rather than managing around it:
- Blood pressure that stays above 140/90 despite intervention
- Erectile dysfunction that persists after estradiol, prolactin and free testosterone have all been corrected
- Any chest pain, arrhythmia or shortness of breath
- Complete loss of morning erections lasting several weeks with clean bloodwork
- Mood changes severe enough to affect your relationships or work
Sexual dysfunction that continues well into PCT and beyond needs a doctor, not another compound. Be straightforward about what you have been taking ā it changes the assessment entirely, and an endocrinologist has seen it before.
ā Tadalafil and Sildenafil are stocked in our Oral Steroids category; Cabergoline and the aromatase inhibitors in the PCT category. Every batch is third-party tested.
Frequently Asked Questions
Is this permanent?
Almost never. On-cycle erectile dysfunction from crashed estrogen, elevated prolactin or high SHBG resolves once the underlying cause is corrected, usually within one to three weeks. What can persist is dysfunction from an inadequate PCT leaving you hypogonadal for months, which is a reason to plan PCT properly rather than a reason to panic mid-cycle.
Should I just add more testosterone?
No, and this is the most common wrong turn. More testosterone means more aromatization and more estrogen to manage, which usually worsens whichever imbalance you already have. If your total testosterone is already supraphysiological, the problem is not a shortage.
Can I run tadalafil for the whole cycle?
Many people do, at 5 mg daily, for the blood pressure and endothelial benefits as much as the primary effect. It is well tolerated long term in clinical use. Watch for back and muscle aches, which are the most common reason people reduce the dose, and be certain you are not taking anything nitrate-based.
Why did this start on Trenbolone specifically?
Trenbolone hits several of these causes at once ā it drives prolactin, it does not aromatize so it contributes no estrogen of its own, it raises blood pressure, and it wrecks sleep. That combination makes it the compound most associated with the problem. Get prolactin and sensitive estradiol tested before adjusting anything.
My bloodwork is perfect but nothing works. What now?
If sensitive estradiol, prolactin, free testosterone and blood pressure are all in range, the cause is likely psychological, sleep-related, or a performance-anxiety loop following the initial episode. Check whether morning erections still occur ā if they do, the physiology is intact. Fix sleep first, and consider that this is one of the situations where the answer is not another compound.
